College Entrance Test (CET) Result and Acceptance Letter signed by the University Registrar.

Photocopy of Birth Certificate issued by the National Statistics Office verified against original NSO Birth Certificate to be submitted upon enrolment or within the semester.

Certificate of good moral character issued by the High School principal or guidance counselor.
